
Modernization is hard. We know this because the same patterns repeat across every sector. Regulated industries, enterprise scale, cloud migrations—it doesn’t matter. Organizations consistently underestimate the complexity and entanglement of their legacy systems, the tribal knowledge and heavily customized business logic embedded in them, and the political negotiation required to shift day-to-day operations and incentives.
Then when risk inevitably emerges, it gets treated as a surprise. Teams scramble to contain issues rather than manage them. By that point, options are limited and costs are rising. What could have been addressed as a known risk becomes a full-blown crisis that drains leadership bandwidth and erodes stakeholder trust.
I’ve watched this happen too many times to count. A program launches with momentum. The business case seems solid. The technology approach seems sound. The team appears capable. Then three months in, the cracks show up. Maybe the legacy system has 200 undocumented integrations instead of the 40 originally scoped. Maybe the data migration is twice as complex due to unexpected source data issues. Or maybe a vocal executive champion changes position once workflows are disrupted. Or a key piece of vendor-promised functionality turns out to have serious constraints.
None of this is unforeseeable. But it becomes a surprise because risk management was treated as a checkbox, not a discipline.
Real risk management must be integral from day one. Too many teams create risk registers to meet a PMO standard, then never use them again. Risk becomes a tab in the RAID log or a bullet on a status slide, not an active practice.
This is one of the biggest gaps between tactical PMs and real consultants. Tactical PMs document risk artifacts. Consultants anticipate risks, evaluate implications, surface options, and facilitate decisions.
What does proactive risk management actually look like?
It starts with identifying specific, high-impact risks, not just generic items like “key resources might leave.” Real examples we’ve seen include:
- Data regulations that complicate discovery, mapping, and archival.
- Legacy integration platforms with unknown compatibility issues.
- Business logic locked in undocumented SQL, with key SMEs long retired.
- Software vendors overpromising during selection, then hedging post-contract.
- Critical functionality delivered via new modules with no track record.
- Custom integrations required because native connectors don’t exist.
These are the types of risks that require real discovery effort. It means listening to people who’ve seen the past failures. It means reverse engineering undocumented systems. It means pushing for clarity on vendor claims. And it means asking, early on, what would trigger an escalation or force a decision.
It also means assigning risk owners. Someone responsible for watching the signals and escalating at the right time. Not just writing “owner: PM” in the log. Actual accountability.
You need defined escalation paths and forums that can act when conditions change. Whether that’s a steering committee or an executive sponsor call, the forum must have the authority to decide, not just discuss. And ideally, sponsors are socialized beforehand so there are no surprises when decisions hit the room.
Because here’s the reality: if you wait until a risk becomes a crisis, your choices narrow. Early on, you can pivot. Later, you’re locked into workarounds, cost overruns, and compromised scope.
There’s another layer too: communication. Executives want to believe their investment is on track. Delivery teams want space to solve problems. So everyone shades green until they can’t anymore. This dynamic is why we’ve seen situations where a dev manager hid a 12-month delay until a month before UAT.
The better approach? Communicate risks with clarity and confidence. One of my long-time client sponsors once said, “Anyone can manage a green project. It’s only when things go wrong that real PMs show up.”
I’ve coached our consultants to always have a plan. That means:
- What’s the risk?
- What’s the impact?
- What’s being done to prevent it?
- What are the options if it materializes?
If you can present that confidently and clearly, stakeholders feel reassured, not alarmed.
Risk framing also matters. “Technical complexity” means little to an executive. “If the data migration takes four months instead of six weeks, we’ll burn $800K in extra contractor time and delay go-live” is a conversation they can engage with.
Risk is especially consequential in modernization programs because of the scale and stakes involved. The decisions made today affect TCO, business value, and platform flexibility for the next decade. One bad call could prevent you from retiring costly legacy systems or meeting regulatory deadlines. The financial and operational exposure is real.
That’s why successful programs build governance rhythms where risk visibility is baked in. Weekly delivery risk reviews. Monthly sponsor briefings. Executive updates with top risks and mitigation actions. And clear lines for escalation when indicators shift.
They also treat risk as a team responsibility. It’s not just on the PM. Everyone should feel empowered to raise concerns. There should be psychological safety to flag potential issues without being seen as negative. And the process should help determine whether something needs program-level visibility or can be handled locally.
Lastly, good programs plan for the “dangling wires,” those dependencies, integrations, and handoffs that all have to be rewired during cutover. That takes real effort and coordination to avoid surprises.
Modernization carries inherent risk. You’re replacing deeply embedded systems, disrupting workflows, changing how people operate. Pretending risk won’t show up is not optimism. It’s negligence.
Managing it proactively, systematically, and transparently—that’s what separates the programs that deliver from the ones that become cautionary tales.
